Since human prolactin was isolated and characterized 13 years ago, the study of the control of prolactin secretion has been intensive. Hyperprolactinaemia is the most commonly identifiable hypothal amic pituitary disorder1,2. The dominant inhibitory nature of hypo thalamic control of prolactin secretion may be the reason that hyperprolactinaemia is such a common condition. During the PClst decade two separate therapeutic approaches to the management of hyperprolactinaemia have been introduced: transsphenoidal selec tive pituitary microsurgery and medical therapy to suppress prolactin secretion with orally active long-acting dopamine agonist drugs. Small prolactin-secreting tumours are treated extremely satisfactorily both with medical and with surgical therapy, both in terms of lower ing serum prolactin levels to normal and in restoring gonadal func tion. However, for the larger tumours, either where the tumour is invasive or the pretreatment serum prolactin level is greater than 1 250ngml- the results of surgery are poor in terms of restoring to normal circulating prolactin levels and gonadal functionJ-s. We now discuss the medical management of hyperprolactinaemia, potential problems during pregnancy and the management of large prolactin secreting pituitary tumours.

A little over 12 years ago I wrote a small volume entitled Infertility. It seemed to me at that time that significant advances in the field called for the publication of such a volume. The following is from the preface to that volume: During the past 15 years considerable progress has been made in the field of infertility diagnosis and management. It is perhaps a paradox that much of this increased knowledge has come about because of Western medicine's preoccupation with the search for a means to control reproduction. As a result, we have achieved new insights into the physiologic mechanisms involved in reproduction, and we have found better methods for measuring physiologic changes in reproductive health and disease. To these advances can be added improvements in the utilization of endoscopic and surgical techniques, in the diagnosis and treatment of infections and endometriosis, and in the treatment of hormonal disorders. During this period, too, through workshops and conferences and in journals and texts, these latest advances have been made available to physi cians, an outstanding example being the two volumes of Progress in Infer tility, edited by Drs. Jan Behrman and Robert Kistner. As necessary as these publications are, they do not offer an overall view of infertility diag nosis and management.

At the midpoint of the 20th century, our knowledge of cancer was based on epide- ology and pathology, and treatment consisted of surgery and radiation therapy. At mid-century, Medawar and colleagues initiated the understanding of transplantation immunology, Farber described the first use of an antifolic drug to treat leukemia, and Jacobson and coworkers described the irradiation-protection effect of spleen cells. These observations opened the door to the development of chemotherapy and tra- plantation in the treatment of cancer. Despite the rapid development of these new disciplines, progress was usually based on empiric observations and clinical trials. The rapid advances in molecular biology at the end of the 20th century mark a new era in our knowledge of cancer. Molecular immunology, molecular genetics, mole- lar pharmacology, and the Human Genome Project are in the process of providing a level of understanding of cancer undreamed of in the past. Optimism is based on the firm belief that understanding at the molecular level will lead to better and earlier di- nosis, to new forms of treatment, and, most importantly, eventually to prevention of many types of cancer.

The International Federation of Fertility Societies XI World Congress on Fertility and Sterility took place in Ireland at the Royal Dublin Society from the 26th June-1st July, 1983.
